Our service accepts patients who may or may not have been sectioned under the Mental Health Act and typically have long histories of admission, medication, discharge and re-admission. Increasingly, patients are being referred to us who are being considered for discharge from hospital by their Responsible Clinician under a Community Treatment Order (CTO). This ensures that our clinical team can work with NHS staff to create a bespoke care, support and treatment package for the individual which will be in place and effectively managed from the day of discharge.
Independent Pathways offers a genuine opportunity to avoid the “Revolving Door” syndrome and avoid some of the pitfalls inherent in existing "step down" services.
Our approach and therapeutic programmes provide Intensive Psychiatric Rehabilitation within each patients own home with patient-centred care plans designed around the individual’s needs.
Due to our unique “dual registration” with the Care Quality Commission (CQC) we are able to deliver
an "in - patient" care service in the community
