The service provides rehabilitation and recovery at the individual’s place of residence in order to reduce, and where possible, eliminate the distress of changing the environment. The care provided by the Independent Pathways is individualised though tailored care packages for service users who require support in the community, helping them to achieve their optimum of development while minimizing the likelihood of a relapse.
The client’s needs are individually assessed and a full community based care plan is put in place. Through a comprehensive assessment it is established what level of support is required in order to ensure safety while promoting independence. The care for our clients is provided for 24hrs where necessary, with the support level ranging from 1:1 to 2:1 at all times. The level of support is regularly revised to accommodate changes in the individual progress and goals and encourage the client to become more independent.
Our service is regulated and overseen by the CQC which ensures that we are able to offer the highest standard of comprehensive support and care in the community.
