Multi-Disciplinary Team (MDT)
Our highly experienced Multi-Disciplinary Team (MDT) provides bespoke risk management with the capacity to increase support to meet the service users’ needs. Care will be jointly reviewed with the Community Mental Health Team (CMHT) providing robust governance arrangements.
Before any commencement of services our clinical team will carry out an extremely comprehensive assessment of each service user referred. Following assessment, the clinical team meet to ensure that a programme can be developed to address the needs of the service user in a way which ideally moves them towards greater independence once they are discharged from hospital.
The service user Care Plan is jointly developed with professionals who are involved in their care and, for those service users who are subject to a Community Treatment Order, jointly agreed with the service users Responsible Clinician.
