Independent Pathways support and care packages are developed in collaboration with the service user ensuring that all of their identified needs are met through person centred planning. Working with social and private landlords we aim to support service users empowering and promoting their independence to enable them to live in their own homes.
Our service provides comprehensive packages of care and support for our service users who have complex needs, support can either be provided in the service user’s own home or in one of our organisation’s projects. We support our service users in accessing suitable education/vocational training and when identified as appropriate support them in identifying voluntary or paid employment.
At Independent Pathways we work with the service user to maintain their existing skills and strive through a programme of meaningful activities, (educational/vocational and social), at the same time encouraging them to develop new life skills enabling him or her to achieve realistic goals, with an aim to them experiencing full community inclusion and integration.
